You can enter the Scheme by filling out the application form to certify your eligibility and sending it with a photocopy of either your birth certificate or passport to:

The form is available by:

• Downloading the application form in either Microsoft Word or PDF format.
• Writing to the Administrator, Insurance Scheme, HSE, Mill Lane, Palmerstown, Dublin 20, requesting an information pack. This pack will include an "application for eligibility" form.
• Emailing insuranceinfo@hse.ie requesting an information pack, which includes the form.
